WEST TORONTO OHT MEMBER
West Toronto OHT (WTOHT) membership expansion occurs through an open process for additional partnership members either as general members or as affiliate members.
Membership is open to all organizations that:
1. Commit to the vision, guiding principles and Patient, Family and Caregiver Declaration of Values of the WTOHT
2. Deliver health or social services to our attributed population
3. Willingness to commit resources to achieve the vision and goals of the WTOHT (e.g. time, money, expertise, staff, space, communications)
4. Agree to work as a collective member of the WTOHT
A key component of our maturity efforts is growth and partnership expansion. Membership applications are reviewed on an ongoing basis and new members are welcomed on an ongoing basis. The WTOHT will do a call out to organizations once a year in Fall.
WTOHT’s Priority Population
Residents living in high priority neighbourhoods (Weston-Mount Dennis, Rockcliffe Smythe, South Parkdale and New Toronto)
Older adults
Individuals managing mental health and addictions
Individuals living with chronic illnesses
General Membership (CDMA Signatories)
Organizations that contribute to the health and wellbeing of the West Toronto community, are signatory members on the Collaborative Decision Making Agreement (CDMA), are involved in decision-making, planning, design, and projects of the OHT and contribute to the success of the OHT by leveraging financial and/or in-kind resources.
If your organization provides services to our priority population, you are encouraged to fill out a General Membership Expression of Interest Form.
Affiliate Members
Organizations that contribute to the health and well-being of the WTOHT community, assist in providing feedback through the WTOHT Members meeting, and/or participate in the planning, design, and projects of the OHT through working groups and/or task forces, and provide input on the recommendations to the OHT.
If your organization provides health or social services to the West Toronto area, but not to the West Toronto priority population, or you are not very familiar with West Toronto partnership, you are encouraged to fill out an Affiliate Membership Expression of Interest Form.
