Step 1

Engaging with providers of seniors services in Mimico. Complete.

• The Frail Seniors Working Group has struck an operations group to engage with providers providing seniors services in Mimico.

Step 2

Understanding the services provided in Mimico. Complete.

• The Frail Seniors team has done a current state analysis of services being provided in the Mimico neighbourhood where we are validating our framework for seniors.

Step 3

Reviewing capacity. Current stage.

• We are in the process of reviewing capacity and developing a basket of services to ensure seniors identified as having rising risk receive the services they need.

Step 4

Education and awareness of this population. Future stage.

• At this stage, we will deliver education and training on the Frail Seniors Framework, outlining how to identify the targeted population of seniors who have rising risk and the associated basket of services for them in Mimico.

Step 5

Evaluation of test of change. Future stage.

• We intend to implement an evaluation framework to understand the impact of this work on seniors’ access to services, their experience and providers’ experience. This stage will also provide us with gaps, lessons learned and an opportunity to implement a quality improvement approach.
