What is Supported Intake?
Supported Intake is a structured process that emphasizes ease of access, thorough assessment, and appropriate service linkage. Our team will guide you to the right resources and ensure a smooth transition into the healthcare system.
BOOK YOUR APPOINTMENT
Do you have an idea of what kind of care you require (eg. family doctor, social services, dietician) but don’t know where to find support? A Community Health Worker or a member of the Supported Intake Team will help direct you.
HEALTHCARE PARTNERS
Looking to direct a patient to the Supported Intake Team? Click the link below to securely refer patients. A Community Healthworker will do the rest!
