Ontario is building a modern, sustainable, and integrated healthcare system. In November 2020 the West Toronto OHT was approved by the Ministry of Health – learn more. In June 2022, we expanded to 35 member organizations.


Our vision

Working together as patients, caregivers and healthcare providers, our vision is to improve the experiences of and outcomes for people living and seeking care in West Toronto for all stages of life by becoming one connected system of health and social care.


Our Strategy

The West Toronto Ontario Health Team is continuing to build a foundation for collaboration and partnerships in West Toronto. Our 2023-24 Operating Plan outlined how we aim to expand the work of our priority areas and equitably involve our community members, local physicians, and member organizations every step of the way.

OUR GUIDING PRINCIPLES HELP move US FORWARD:
